SNP Analysis Feedback Form:

About this form:
This is a simple form that must be completed (as mentioned in the initial contract) by all users of a FUGE core-facility after fulfillment of a project.

The form is separated into 4 blocks, dealing with a separate stage of the analysis process and one general part in the end. There are five alternative answers to most of the questions, ranging from 1 (very good/very likely) to 5 (very bad/very unlikely). A few questions are of Yes/No type. At the end of each block, there is a box for comments. Please use this box frequently since this is a good way for us to get feedback on our services. Positive feedback is also welcome.
